This form is a legal document that affects your rights. By signing or electronically agreeing to this form, you acknowledge that you have read, understood, and voluntarily agree to the terms and conditions outlined below.
- Acknowledgement of Services
I understand that I am voluntarily choosing to participate in consultations, teachings, workshops, programs, or healing methods provided by Olga Dakiv (the “Facilitator”), which may include her unique healing method, insights, and spiritual guidance. I acknowledge that these services are educational and spiritual in nature, and are not a substitute for professional medical, psychological, or legal advice, diagnosis, or treatment. - Assumption of Risk
I understand that consultations, teachings, programs, and healing methods offered by Olga Dakiv are intended to support personal growth, well-being, and spiritual development. I recognize that any activity involving health, spirituality, or personal growth carries inherent risks. By choosing to participate, I voluntarily assume full responsibility for any risks, injuries, or outcomes that may result, whether expected or unexpected, from my participation in any consultation, teaching, program, or healing method. - Waiver and Release of Liability
In consideration of receiving services from the Facilitator, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, discharge, and hold harmless Olga Dakiv, her business, employees, contractors, heirs, and assigns from any and all claims, demands, damages, actions, or causes of action arising out of or connected with my participation in these services. This includes, but is not limited to, any claims for physical, mental, emotional, financial, or spiritual outcomes connected to the use or misuse of the services, teachings, or healing methods provided. - Informed Consent
I understand and agree that:
• No guarantees have been made regarding results or outcomes.
• All information shared is for educational and spiritual purposes only.
• It is my responsibility to seek professional medical, psychological, or legal advice for any condition or concern requiring such expertise.
• I am fully free to accept or reject any guidance, teaching, or method offered. - Covenant Not to Sue
I agree that I will not initiate, join, or participate in any lawsuit, claim, or legal action against Olga Dakiv, her company, contractors, or agents arising from my participation in her consultations, teachings, programs, or healing methods. - Refund Policy
I understand and agree that all consultations, teachings, programs, and books are non-refundable. If I need to reschedule a consultation, I may use the rescheduling link provided in my confirmation email. - Acknowledgement of Understanding
By electronically checking the agreement box, I confirm that:
• I have carefully read this Waiver, Release, Assumption of Risk, Informed Consent, Covenant Not to Sue, and Refund Policy.
• I fully understand its contents.
• I voluntarily agree to its terms with full knowledge of its significance.

How Long Is Consultation And What's The Price?
1 hour @ $250 USD
Consultation is nonrefundable.
If you need to reschedule, use the link from the original confirmation email and pick a new date and time. You can reschedule any time up to 48 hours before the original consultation date.
How Is A Consultation Done?
Once you complete your booking online, you'll receive an email with date, time and private Zoom link to join the private consultation, so make sure Zoom is installed on your device ahead of time. You can download it for free here:
